Submitting an Investigative Services Requests Forms

Please fill out the current 10-8 Investigative Services - Investigative Services Request Form in either fillable .pdf or .doc formats as completely as possible.

You may upload this information via this portal or send as an attachment to pi@defendit.ca.

The more information that is provided on the investigative services form to our investigators, the greater probability of a positive hit for the information you are seeking.  Less information provided to us may result in higher investigative costs as more queries may need to be conducted to acquire the information that you are seeking.

We will require at least ONE of the "minimum" search parameters to locate ANY subject which includes:

• Date of birth
• Driver's licence number
• ANY last known address of the subject
• Licence plate associated to the subject
• Approximate age

All searches must be lawful searches pertaining to litigation, process serving or a missing person complaint.  For example, with all Investigative Services Request Forms, you MUST include a copy of:

• A court order or tribunal order
• A copy of any court application of litigation filing such as a Plaintiff's Claim or a Statement of Claim
• A copy of any tribunal application such as a Landlord and Tenant action or a Human Rights application
• A collision report
• A copy of a police report obtained through Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c. F.31 or Municipal Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c. M.56

If the request is for pre-litigation purposes or there is a proposed lawsuit or process serving in the near future, then we would require, on letterhead why you are seeking this personal information, and the reason for this request (i.e. process serving or litigation).

If we do not receive this information accompanied with your Investigative Services Request Form, we will not process your request.
